Clickbait: A Rare Case of Monomorphic Ventricular Tachycardia and Clicking Sensation without Infectious Presentation Revealed as Infective Endocarditis with Aortic Root Abscess

Context: Presenting symptoms of infective endocarditis (IE) typically includes infectious signs such as diaphoresis, fever, and malaise. Pathogenic microorganisms implicated in IE may form vegetations, collection of debris that may subsequently embolize. Notable complications and concomitant sequelae of systemic embolization from resulting from valvular vegetations include stroke, acute renal failure, and cutaneous manifestations. Valvular dysfunction warranting surgical repair does not preclude redo-repairs from subsequent emboli, highlighting the extensive propensity of IE to cause acute decompensation across multiple domains. A presenting feature of arrhythmia stemming from valvular dysfunction have been appreciated in the literature. However, literary accounts of ventricular tachycardia (VT) as the resultant arrhythmia are scant. Moreover, these accounts usually include concomitant infectious presentation. We report the case of a 36-year-old male with a past medical history of Bartonella spp. endocarditis and aortic valve repair who appeared to the emergency department with acute onset monomorphic VT. Complicating his presentation was a reported sensation of clicking in his surgically repaired valve while lacking infectious symptoms. Arrhythmia was treated with further workup delineating S. epidermidis vegetations and aortic root abscess successfully treated with antibiotics and ICD placement.

Post Abortion Women's Life Style: Impact of Counseling Based on PLISSIT Model

Context: Background: Counseling on post-abortion care is vital for reproductive health, utilizing structured models for informed decision-making. The PLISSIT model (Permission, Limited Information, Specific Suggestions, Intensive Therapy) offers a stepwise approach to address women's concerns regarding psychological, emotional, and social complications post-abortion. Its effective implementation can mitigate post-abortion complications and improve women's knowledge, emotional adaptation, and lifestyle. Aim: The current study was conducted to evaluate the impact of counseling based on PLISSIT model on post abortion women's life style. Subjects & Methods: Design: A quasi-experimental (pre- and post-test) research design was used. Sample &Settings: A convenient sample of 92 women who had abortions affiliated obstetrics & gynecology unit at Beni-Suef University Hospital. Tools: (1) women's knowledge regarding abortion and management sheet; (2) women's lifestyle after abortion Questionnaire; (3) information technology Questionnaire; (4) abortion counseling based on the PLISSIT model. Results: 15.2% of the studied women had satisfactory knowledge regarding modern educational information on abortion pretest which improved posttest to become 80.4%. Also, 88% of the studied women had unhealthy lifestyle regarding weight and exercise which improved posttest to become 27.2%. Moreover, 3.3% of the studied women had good level of impact of social networking sites after abortion which improved posttest to become 76.1%. The mean total knowledge of the studied women about abortion during pretest was 20.73±6.33 which improved posttest to become 34.04±6.76. Conclusion: In the pretest, no correlation was found between total knowledge, lifestyle, and the impact of social networking sites on women post-abortion. However, the posttest revealed a correlation between total knowledge and the impact of social networking sites. Furthermore, positive correlations were identified between total knowledge and overall lifestyle, and between overall lifestyle and the total impact of social networking sites on women post-abortion. Recommendations: Developing an educational program to enhancing women’s knowledge regarding the using of social media properly, maximizing its advantages, introducing women to the most trustworthy and legitimate social media sites, and showing them how to use these resources to improve women’s life style after abortion.

Correlation between Women’s Knowledge, Life Style and Impact Level of Social Networking Sites on Women After Abortion Using Counseling Based On PLISSIT Model

Context: Background: Abortion is the termination of a pregnancy prior to fetal viability, often presenting a woman with physical and emotional challenges. Nurses play a crucial role in post-abortion counseling, aiding in recovery, lifestyle improvement, and providing psychological support to women and their families. The PLISSIT model offers a framework for healthcare providers to implement effective strategies for addressing post-abortion life adjustments. Aim: The current study was conducted to evaluate correlation between women’s knowledge, life style and impact level of social networking sites on women after abortion using counseling based on PLISSIT model. Subjects & Methods: Design: A quasi-experimental (pre- and post-test) research design was used. Subjects & Settings: A convenient sample of 92 women who had abortions affiliated obstetrics & gynecology unit at Beni-Suef University Hospital. Tools: (1) structured interview questionnaire. (2) women's knowledge regarding abortion and management. (3) Women's lifestyle after abortion. (4) Information technology. (5) post-abortion counseling based on the PLISSIT model. Results: mean age 28.13±5.824 years. The mean of the total general knowledge of the studied women during pretest was 20.73±6.33 regarding general knowledge about abortion which improved posttest to become 34.04±6.76. Total life style improved from 38.17±10.99 pretest to 68.33±10.38post test. Total impact of social networking sites on women after abortion improved from 8.15±3.05 to 14.69±3.25 after implementation of counseling. There was a statistically significant improvement among the studied women regarding knowledge; life style sub-items on abortion posttest and total impact of social networking sites on women after abortion, as well (p≤0.01). Conclusion: There was no correlation between total knowledge, lifestyle and impact level of social networking sites on women after abortion during pretest. Also, there was negative correlation between total knowledge and impact level of social networking sites on women after abortion during posttest. While, there was strong positive correlation between total knowledge and total lifestyle level and between total lifestyle level and total impact level of social networking sites on women after abortion. Recommendations: Developing strategies to facilitate women's understanding and support lifestyle adjustments after abortion by applying this study to a large sample in inpatient and outpatient departments.
